Type 2 DESMOND Group Education


DESMOND Diabetes Education and Self-Management for Ongoing and Newly Diagnosed (DESMOND) is a National Diabetes Structured Education Programme for people with Type 2 diabetes, whether newly diagnosed or who have had their Diabetes for more than one year.

The aim of the programme is to increase an individual's knowledge of Type 2 Diabetes, share self-management skills, have a better understanding of poor glycaemic control and come away from the session with an understanding and empowerment to set clear, effective goals and improve your overall health and wellbeing.

Newly Diagnosed module for patients up to 12 months post diagnosis.
Foundation module for those with established diabetes.
One day courses including Saturdays for 10-12 people with diabetes plus a partner, family member or friend.

Courses are run in Brent in Wembley, Willesden, Harlesden, Kingsbury and Kilburn.

The standard courses are run in English, but on-demand courses are also held in other languages such as Gujarati, Hindi, Punjabi, Tamil, Urdu and Arabic.

For people who do not speak English or those with a disability or who are housebound, Hillingdon offers "mini" DESMOND, providing a one to one session for one hour with a diabetes specialist nurse.

What does DESMOND entail?
DESMOND is a group course for up to 10 people with type 2 diabetes. The course is built around group activities, with individuals able to speak to an educator. It is a one day course, offered on days to suit you. Each course is attended by about 10-15 people with type 2 diabetes. The local venues for DESMOND in Brent are; Monks Park Primary Care Centre, Willesden Centre for Health and Care and Wembley Centre for Health and Care.

DESMOND is a one-day health education programme for people in Hillingdon with type 2 diabetes and who are in their first year of diagnosis.
It offers a way of finding out more about type 2 diabetes, a resource to help individuals to manage the changes that diabetes will bring to their life, and an opportunity to meet and share experiences with others.
The programme is delivered as a one-day session with trained facilitators using specially developed resources It takes place at several locations in Hillingdon in groups of 10-12 people.

For people who do not speak English, or those with a disability or who are housebound, we now offer mini DESMOND with a one-to-one session for one hour with a diabetes specialist nurse.

Following a general practitioner (GP) referral, individuals are invited to attend a venue of their choice and are encouraged to bring along a friend or relative for support. Those who attend the course will learn all about diabetes and how management of the following can improve wellbeing:

  • Exercise
  • Blood glucose monitoring
  • Medication
  • Ways to improve your diet including all about fats
  • Carbohydrate and sugar content in food and drink
  • Complications from diabetes and how to prevent them
Courses are scheduled on a regular basis with a choice of location, including some weekend sessions.
