For people with raised blood glucose levels (Non-Diabetic Hyperglycaemia). This GP referral program comprises diet and lifestyle education, help with weight loss and a personalised exercise programme

• One to one initial assessment, lasting one hour

• Ten weekly group sessions that include healthy lifestyle advice and physical activity (two hours each session)

• Four monthly group sessions that include strategies to help you maintain your lifestyle changes

• Three one-to-one progress review sessions lasting an hour at three, six and nine months

Move Away From Pre-Diabetes M.A.P

Brent has developed its own intensive lifestyle intervention for individuals who are identified as being at high-risk of developing diabetes in the future.
The Move Away from Pre-diabetes (MAP) programme uses an intensive behaviour change approach to help residents make sustainable changes to the way they eat and exercise.
The overall aim of MAP is to reduce the incidence of diabetes and cardiovascular disease in the Brent community.

Participants are referred onto the programme following screening by the GP (NHS health check) that fulfils the following criteria:
• You are aged between 40-74 years
• Tests show that you have either:

The programme offers:
• Three months free gym membership to support your lifestyle changes
• Weekly one hour group exercise sessions for twelve weeks
• Weekly one hour and fifteen minute nutrition sessions with a registered dietitian for seven weeks
• Specialised cooking workshops
• Individual appointments with a dietitian
• Follow-up support for a total of eighteen months
The programme is engaging, motivating and an excellent opportunity to improve health and to reduce the risk of chronic disease. The service is inclusive and meets the needs of the diverse community of Brent by offering:
• An interpreter if required
• A choice of locations – Willesden Sports Centre, Charteris Sports Centre or Vale Farm Sports Centre
• After-hours group sessions, for participants that work full-time
